Children with serious illness face an uncertain future, and families must prepare for many eventualities. Today’s health care environment is specialized, with many professionals contributing expertise to the care of individual patients. Coordination of care for patients with complicated illness is an important goal in assuring the best quality of life for them and their families. This conference will address the role of families and caregivers in advance care planning for children with life threatening illness using both didactic and interactive learning methods. It will be of interest to families, primary and specialty care clinicians, therapists, educators, ethics committee members, and all who have chosen work role that strives to preserve and restore the health of others.
